Terms, Conditions and Medical Release Form

• Fill out both forms.
• Make Checks out to CMP Wrestling.
• Mail payment and forms to: P.O. Box 1964, Marietta, GA 30061
• You will need to pre-register 2 weeks prior to each camp to get a free t-shirt.  We will have to have minimum number of registration to order shirts.
• If your child is receiving free or reduced lunches through the National School Lunch Program (NSLP), please click ASSISTANCE and DONATIONS .
• If your child is not receiving free or reduced lunches and still need assistance in paying, please click FUND RAISERS.
